I’m guessing you’re referring to a CT scan or an MRI. In scans of the spine or joints, the radiologist I work with often says this…. It signifies that the soft tissues seem normal.
Definition of soft tissues: Soft tissues are tissues that link, support, or surround other structures and organs in the body. Muscles, tendons, fibrous tissues, fat, blood vessels, nerves, and synovial tissues are all examples of soft tissue. en.wikipedia.org/wiki/Soft%5Ftissue

Sounds like a statement from a medical checkup of some kind – either a doctor evaluating an injury or an autopsy. This means that there is nothing unusual or unusual about the location they are looking at. Any portion of the body that isn’t bone or cartilage is considered soft tissue.

“The soft tissues are unremarkable” suggests that nothing seems to be amiss.

When physicians say “unremarkable,” they imply “no comments” or “nothing to say about it.”
This was most likely doctor’s notes following a medical examination.
